Case Presentation.

Blurred vision as presenting symptom of brain tumor

Case history

A 75 year old women presents with decreased vision in the right eye for the last three months. 

Past medical history:

Arthritis and low blood pressure.

Past ocular history:

Bilateral cataract extraction several years ago.

Exam:


Vision with current glasses is count fingers in the right eye, and 20/40 in the left.


Dilated retinal exam:

Dense epiretinal membrane through central macula in the right eye.

Right eye:    
   



Fluorescein angiogram:

Some sublte leakage of the parafoveal vessels in the right eye.

Right eye at about 30 seconds:    
   
     
Right eye at about 5 minutes:    
   

 

Optical coherence tomography:

Increased retinal thickening with prominent ERM.

Right eye:  
 


Diagnosis:

The patient has a dense epiretinal membrane in her right eye with corresponding drop in vision.

Plan:

Vitrectomy with membrane peeling.

Follow up:

Three months follwing surgery, patients vision is 20/60 in the right eye.

Fundus photos show scar tissue removed.

OCT reveals some residual retinal thickening, but no significant epiretinal membrane.
